Pelvic Ultrasound: What Patients Should Know

A pelvic ultrasound uses sound waves to image pelvic structures — including the uterus, ovaries, and bladder in women, or the bladder and prostate region in men. It does not use ionizing radiation. Preparation requirements vary by exam type.

Educational content only. DOD does not perform or interpret pelvic ultrasound. Obstetric and gynecologic emergencies require immediate evaluation by a qualified licensed provider.

Quick Answer

A pelvic ultrasound uses sound waves (no radiation) to image the uterus, ovaries, bladder, and surrounding pelvic structures in women. Transabdominal pelvic ultrasound requires a full bladder. Transvaginal ultrasound (TVU) uses a small intravaginal probe and provides improved resolution of the uterus and ovaries and requires an empty bladder. Provider orders are needed at most facilities. DOD can review pelvic ultrasound order requests for eligible patients in supported states.

Types of Pelvic Ultrasound

Transabdominal Pelvic Ultrasound

Preparation: Requires full bladder

Probe placed on abdomen over the lower pelvis. Full bladder serves as acoustic window. Good for overall pelvic survey, large uterus/masses.

Transvaginal Ultrasound (TVU)

Preparation: Requires empty bladder

Small probe inserted vaginally for closer proximity to uterus and ovaries. Higher resolution for small structures. Used for early pregnancy, follicle monitoring, small uterine/ovarian pathology.

What Pelvic Ultrasound May Evaluate (Women)

Uterine size, shape, and wall (myometrium)
Endometrial thickness and texture
Ovarian size, follicles, cysts, masses
Fallopian tubes (limited visualization unless abnormal)
Bladder size and wall
Free pelvic fluid (peritoneal fluid)
Pelvic masses or adnexal pathology

Educational only. Clinical appropriateness determined by a licensed provider.
