An X-ray (radiograph) is one of the oldest and most commonly used forms of medical imaging. It uses electromagnetic radiation to create images of internal body structures — particularly bones and other dense structures — quickly and at relatively low cost.

How X-Ray Works
An X-ray machine generates a controlled beam of X-rays directed at the body. A detector (film or digital sensor) on the other side captures the image. Different tissues absorb X-rays to different degrees — dense bone absorbs more and appears white (radiopaque), while air absorbs very little and appears black (radiolucent). Soft tissues produce various shades of gray in between.
Multiple views are often taken (e.g., frontal and lateral chest X-ray) to provide a more complete assessment. According to the FDA, modern digital X-ray systems deliver lower radiation doses than older film-based systems while maintaining or improving image quality.

Vertebral alignment, disc spaces, fractures, scoliosis assessment
Extremities
Suspected fractures, bone alignment, joint spaces in hands, wrists, arms, legs, ankles, feet
Pelvis / Hip
Hip fractures, joint alignment, pelvic trauma assessment
Abdomen
Bowel gas patterns, foreign bodies, calcifications — limited soft tissue detail
Sinuses / Skull
Sinus congestion, skull fractures (though CT is preferred for complex cases)
What X-Ray Cannot Show Well
- 1Soft tissues — ligaments, tendons, cartilage, and most muscles are poorly visualized on standard X-rays.
- 2Early or subtle fractures — MRI or CT may be needed when clinical suspicion is high and X-ray is negative.
- 3Spinal cord or nerve root pathology — MRI is preferred.
- 4Internal organ detail — abdominal organs (liver, spleen, kidneys) are not well visualized on plain X-ray.
Provider Order Requirement
Most outpatient imaging facilities require a provider order before scheduling an X-ray. DOD can review X-ray order requests for eligible patients in supported states when medically appropriate.
