Do Not Use This Service for Emergency Symptoms

Call 911 or go to the nearest emergency room immediately if you have:

  • • New bowel or bladder incontinence or retention
  • • Saddle anesthesia (numbness in groin/inner thighs)
  • • Rapidly progressive weakness in both legs
  • • Fever with severe back pain (possible spinal infection)
  • • Back pain following major trauma or fall
  • • History of cancer with new or worsening back pain

These symptoms may indicate serious conditions requiring immediate emergency evaluation. Diagnostic Orders Direct is not an emergency service.

Who This Is For

This patient pathway is designed for people with back pain who need a provider order to schedule imaging — and can't get one quickly through traditional channels:

Persistent lower back pain lasting 4–6+ weeks

Back pain with radiating leg pain, numbness, or tingling (sciatica)

Prior X-ray was inconclusive or normal but symptoms persist

You've tried physical therapy or conservative treatment without relief

You found an affordable cash-pay imaging center but need a provider order

You don't have a primary care doctor or can't get a timely appointment

Your PCP wait time is weeks and imaging is already recommended

You need imaging before a specialist or orthopedic appointment

What the Provider Reviews

During your 10-minute virtual consultation, the licensed medical provider will evaluate:

Pain location and character

Where exactly is the pain? Sharp, dull, aching? Does it radiate into your buttocks, hips, or legs?

Duration and trajectory

How long has this been going on? Is it getting worse, stable, or improving?

Neurological symptoms

Any numbness, tingling, or weakness in the legs or feet? These suggest nerve involvement.

Prior treatment and care

Have you tried physical therapy, chiropractic, medications, or injections? What helped and what didn't?

Prior imaging results

Have you had prior X-rays, CT, or MRI? If so, what did they show? The provider will account for this in their assessment.

Current medications and medical history

Relevant diagnoses, surgeries, and medications that may affect the clinical picture.

Desired facility and study type

Which imaging center are you planning to use? Which spine region do you believe is most symptomatic?

Spine Region: Lumbar, Cervical, or Thoracic?

If you're unsure which spine region needs imaging, that's completely normal. The provider will help determine the appropriate region based on your symptoms. As a general guide:

Lumbar Spine (Lower Back)

Lower back pain, sciatica, radiating pain into the hips, buttocks, or legs, numbness/tingling in legs or feet.

Order Info

Cervical Spine (Neck)

Neck pain, arm pain, shoulder pain, numbness/tingling in hands or fingers, headaches from the base of skull.

Order Info

Thoracic Spine (Mid-Back)

Mid-back pain, pain that wraps around the ribs, difficulty twisting or bending at the middle back.

Order Info

The provider will specify the correct spine region and imaging protocol on your order based on your clinical presentation — not patient preference alone.

How the Order May Be Used

1

Order issued to you digitally

If your request is medically appropriate, you receive the provider order electronically within 1 business day.

2

Present to your chosen imaging center

Take the order to any accredited imaging facility that accepts provider orders. Confirm facility acceptance before scheduling.

3

Schedule and complete your imaging

The facility will perform your spine MRI based on the protocol specified in your order. You pay the facility's self-pay or insurance rate directly.

4

Results sent to the ordering provider

Imaging results and radiology reports are typically sent to the ordering provider. Discuss findings with your primary care doctor, specialist, or another provider.

Frequently Asked Questions

Before Searching for a SesameCare Doctor or Amazon Doctor for Back Pain Imaging

Patients with back pain often search online for a doctor who can order MRI or other spine imaging. DOD can review non-emergency back-pain imaging requests in supported states. Before booking any online doctor visit for back pain imaging, please read the emergency warnings above — bowel or bladder changes, saddle numbness, progressive leg weakness, fever with back pain, major trauma, and rapidly worsening neurological symptoms require emergency evaluation, not a virtual order review.

For patients with persistent, non-emergency back pain who need a provider order to schedule imaging at a specific facility, DOD's review includes your symptom history, prior imaging, prior care, chosen facility details, and clinical appropriateness. The order is not guaranteed. Facility acceptance varies. Truthful symptom reporting is important for accurate provider review.

DOD does not diagnose disc herniation, radiculopathy, stenosis, fracture, arthritis, or other conditions. Diagnosis is the role of the radiologist, treating provider, or specialist after imaging results are reviewed.
